The main route climbs from Milan past Lecco and Lake Como to Chiavenna, then over the Maloja Pass (1,815 m) into the Upper Engadin. A longer alternative runs via Tirano and the Bernina Pass (2,328 m).

Remarkably for this altitude, both passes are kept open all year. The Maloja Pass (1,815 m) — rare for an alpine pass — can see temporary closures of a few hours up to a day after heavy snowfall; the Bernina Pass (2,328 m) is the highest road pass in Switzerland kept open year-round. In winter, winter tyres are mandatory and snow chains can become compulsory on both.

Train from Milan to St. Moritz — scenic, but long

Let's be honest: the rail route is gorgeous. The Bernina line of the Rhaetian Railway has been a UNESCO World Heritage route since July 2008, climbing to 2,253 m at Ospizio Bernina. But there is no direct train from Milan. You first take the Trenord RegioExpress from Milano Centrale to Tirano — 2h30 direct, about 15 trains a day (first around 06:20, last around 21:20), tickets from roughly €13 — then change onto the RhB for another 2h15–2h30 up to St. Moritz.

On the Swiss leg you choose between the panoramic Bernina Express, which requires a seat reservation of CHF 32 per leg on top of the ticket, and the regional RhB trains on the same line — no reservation, around CHF 30–33 in 2nd class. Either way, the realistic total from Milan is about 5h30–6h door to platform, luggage in hand at the Tirano change. Coming from Malpensa, add the Malpensa Express to Milano Centrale first (52 min, €15 adult).
