From Milan you take the A9 motorway towards Como and cross the border at Chiasso-Brogeda — the main motorway crossing between Italy and Switzerland, open 24/7 — then the Swiss A2 to the Lugano exits. Swiss motorways require a vignette (CHF 40): with a transfer, the driver takes care of it.

City centre to city centre, travelling light, the direct train is excellent. The maths change when the schedule is not yours: fixed-time meetings, offices away from the centre (Lugano's station sits on the hill above the town), evening returns after the last useful departure, luggage and groups.
